rather than Eddie Smith. Put any nicknames in paranthesis: William (Eddie) Edward Smith.
Women who list a current married last name, please indicate your maiden name in high
school on the next line. (These records are cross-checked with names on grad lists
and yearbooks)
When listing an email address please do 2 things:
1) If you use spam blockers or other kinds of mail filters (particularly AOL
subscribers who use this option), please list us as one of your assigned
correspondents from whom you accept mail. 2) Please add us to your
personal addressbook so that when you move or change servers, you will remember to notify
us just as you do your other friends and relatives. Both of these problems result in
lots of returned email and your class can't find you.

A permanent address (parent...other relative) would be helpful in locating you should
you move. Also, please list any brothers, sisters, parents etc. that also attended CHS and
their respective class years. This is helpful for other classes who are looking for
missing classmates. Even if they don't check in, CHSAA can locate them by inquiring
through you.

Coronado Schools Foundation
If you would like information regarding the Coronado Schools
Foundation (CSF), a non-profit 501 (c) (3) organization which
provides critical funds for all Coronado school levels, click 'yes' to release your
mailing address to them.

We here at CHSAA do not do any fund-raising - membership is
automatic. CSF was founded in the 1970s by interested and concerned residents to
provide additional, critical funds for all grades/classes in the School District.
Alumni are encouraged to support CSF by attending some of their events throughout the
year. Remember...the 'All-Class Reunion', which is so popular with alumni, is
sponsored by CSF! Also, many other CSF activities are highlighted in the
Coronado Journal-Eagle, as well as their own newsletters. Anyone can join CSF
to support the school system. You do not have to have attended school here.
Check that your information is correct and then click 'send form'
ONCE. It should open your email program and automatically paste the form data into
the body. IT WILL NO LONGER AUTOMATICALLY ADDRESS IT BACK TO US.
Due to spam, we request you manually type in our address.
After you have clicked open your mail program,
use this address. If for any reason this new form method doesn't work, please email
us and let us know.
(If you are on AOL...this process may have to be handled
manually. Sometimes they come in blank.
In accordance with the 1974 Privacy Act,
you must authorize CHSAA to share this information with your Class Secretary. We do NOT
give out your personal information to anyone unless we have a copy of your release
on file. You must check yes below, allowing us to send this data to your
class. Even if you think we (or your class) have your current data, please fill out
the form to make sure we have your release on file. Otherwise the data
merely accumulates and is of no use to anyone. This electronic form is the
same form we use at the All-Class Reunion on July 3. If any of your data is
private or unlisted, you may leave the field blank, indicate N/A for not applicable, or
write "unlisted" to alert your class to this added privacy concern. You may also
use the "Comments" section for this. We will honor all such requests.
